Assumption of Risk

I understand that baseball and athletic training involve inherent risks, including running, throwing, batting, catching, sliding, physical exertion, contact with balls, bats, equipment, playing surfaces, other participants, and environmental conditions. Injuries may occur despite appropriate coaching and safety precautions.

I voluntarily authorize the registered participant to participate in Cerbo Baseball Academy activities and knowingly accept the ordinary and inherent risks associated with participation.

Emergency Medical Authorization

If I cannot be reached in an emergency, I authorize Cerbo Baseball Academy personnel to seek reasonable emergency medical assistance for the participant. I understand that I remain responsible for medical expenses incurred on the participant's behalf.

Release

To the extent permitted by applicable law, I release Cerbo Baseball Academy, its owners, coaches, instructors, agents, and facility partners from claims arising from the ordinary and inherent risks of participation, except to the extent a claim cannot legally be waived.

Rules & Conduct

The participant will follow reasonable safety instructions, use equipment appropriately, and conduct themselves respectfully.
